Uso de cumarina en pacientes ambulatorios adultos de un hospital público en Córdoba, Argentina.

Resumen

RESUMEN:

Antecedentes y objetivo: Cumarina es indicada para tratar el linfedema, se ha generado controversia respecto a los beneficios del tratamiento, y sus posibles efectos adversos (EA) hepáticos.

Cápsulas de cumarina (CC) de 200mg son elaboradas en el área de farmacia de un hospital público de Córdoba, se ha detectado un incremento significativo en su dispensación a pacientes ambulatorios en los últimos años, sin reportes de sospechas de EA.

El objetivo fue describir su uso en pacientes ambulatorios adultos atendidos en el Servicio de Flebología y Linfología (SFL) del hospital durante el año 2016.

Materiales y métodos: estudio transversal observacional y descriptivo a partir de historias clínicas (HC). Los datos fueron registrados y analizados en Microsoft Excel®. La seguridad se analizó mediante tres algoritmos.

Resultados: 58 pacientes tenían diagnóstico de linfedema, 49 femeninos y 71 años promedio. El número de consultas promedió 2,8±1,6. Los tratamientos fueron de 200 mg/día y 6±3,8 meses, con tratamientos complementarios: elasto-compresión, cumarina tópica y diosmina-hesperidina oral, entre otros. Además, un promedio de 3,2±1,7 medicamentos por paciente para las patologías base. El 71% tuvo buena respuesta al tratamiento. Tres, presentaron aumento de enzimas hepáticas y otros dos, hepatitis.

Conclusiones: permitió conocer el estado de situación en el uso de CC 200 mg en pacientes ambulatorios adultos atendidos en el SFL del hospital durante el año 2016, anteriormente inexistente.

Los resultados en efectividad fueron en su mayoría buenos o superiores. Los casos de sospecha de EA estuvieron en el límite superior como lo reportado en bibliografía.

ABSTRACT:

Background and objectives: Coumarin is indicated for treating lymphedema. There is controversy about benefits of treatment and potential hepatic risk.

Coumarin 200 mg capsules (CC) are prepared at a public hospital´s pharmacy unit. Their dispensing to adult ambulatory patients has increased in the last years without any suspected adverse event reported.

The objective was to describe its use in adult ambulatory patients who assisted to the Phlebology and Lymphology Service (PLS) at the National Clinical Hospital (NCH) during 2016.

Materials and methods: transversal observational descriptive. Data were retrieved from the patients' medical records. The patients, treatments and response to treatment details were registered and analised using Microsoft Excel® spreadsheets. Three algorithms were used to analyses the safety of the medicinal product.

Results: there were 58 patients with lymphedema, 49 female and 71 years old average. The medical consultations per patient averaged 2.8±1.6. The prescribed amount was 200 mg/day and the duration of treatment was 6.0±3.8 months. The most common complementary treatments used were compression therapy, coumarin topical use and diosmin-hesperidin oral use. Along with the lymphedema treatment, patients used medicines 3.2±1.7. Seventy-one percent patients had good response to treatment. About security, 3 patients had increased hepatic enzyme and 2 patients presented hepatitis.

Conclusions: the data collected made possible to know the use of CC 200 mg in adult ambulatory patients who were treated in the PLS at the NCH during 2016, previously unknown.

The responses to treatment were mostly good or superior, but there were adverse effects suspected cases.
